Step 1: Choose the way of downloading the lists of applicants

Choice of the most appropriate form (recommended)

1. Type of appendix to the list of applicants:

	standard appendix intended for certificate issuing
	change appendix intended for applicant's personal data and/or certificate data change

2. Certificate type, which shall be issued or its data changed:

	qualified certificate for the verification of the electronic signature of employee
	qualified system certificate for the verification of the electronic mark of organization

(It is possible to check one or both boxes.)

What is the difference between the certificate for the verification of the electronic signature and electronic mark?

Displaying the list of all accessible forms (for advanced users)
